Improve Health Outcomes

Overview

Everybody wants the very best health and wellbeing for our families, friends, communities and for themselves. So when people need to access health and care services, they should provide excellent care and the best outcomes.

Preventing mental and physical health conditions before they develop is better for everyone. It helps people to be healthier for longer and reduces pressure on health and care services so that everyone can get the right quality care, treatment and support when they need it most. Focusing on preventable conditions, targeting those most at risk, will also help to reduce differences in health outcomes.

The new Integrated Care System is a partnership that brings NHS services together with local authorities and other local partners to collectively plan health and care services to meet the needs of the local population. As part of the developing Integrated Care System arrangements, there is an opportunity to ensure Wirral’s Place Based Partnership builds on the collaboration achieved through the pandemic and has a clear focus on reducing health inequalities in Wirral.
